Bramshaw House is a residential care home in Worthing, West Sussex for ten people with learning and physical disabilities. It is a converted building and accommodates ten studio rooms over two levels that are fully equipped to meet the needs of people with learning and physical needs.

Located in the lovely seaside town of Worthing, Marlow provides residential care for 15 people with a range of complex health needs. As well as a communal patio and garden area, the home also benefits from a large sitting room, dining room and peaceful lounge.

Our referral and assessment process

We place each individual at the heart of their care, taking time to understand their personal history and story. Our assessment process is uniquely tailored to ensure the package of care we propose will be as unique as the person we support.

Introduction

We are incredibly proud of the high standard of our services and this begins as soon as you contact us with a referral. We recognise the different degrees of urgency and are able to react as necessary to each individual’s needs, whether immediate or requiring longer term planning. Each referral and assessment will be professionally managed with respect and skilled therapeutic input to ensure our support has taken into account all areas of an individual’s needs.

What happens when you refer someone?

We work with specialist teams to gather all information regarding each person’s needs and specific requirements. Our holistic approach means we are assessing the whole person, rather than just their medical needs. Our experienced team will consider the desired location and type of service required, timelines involved and, most importantly, the difference we can make to enhancing the opportunities and life skills of each individual.

Who does the assessment?

We have a highly experienced team involved in our assessment process. Typically, the Area Manager and Home Manager of the preferred home will attend the assessment and would then refer to any necessary specialists if required.

Who in involved in the assessment?

Part of the transition planning process is the development of care-plans and risk assessments for each individual. These are produced from a collaboration with the registered home manager, their team and external therapeutic professionals where necessary. These documents support visits during the individual’s transition and are fully in place from the moment a person moves in.

How long does the assessment take?

This is entirely dependent on the individual’s circumstances and needs. We ensure that regardless of timeframes the assessment process always maintains the highest possible standards.

Are reports generated?

We produce a therapeutic care pathway that accompanies any placement offer. This document contains information about Progress Housing, our assessment findings and our offer and recommendations related to the placement offered. Also included are the level of staff support and costs.

Who makes the final decision?

We believe in partnership with the individual and their advocates, so ultimately the decision is a joint one, made by the professionals responsible for the funding of the placement, the individual themselves and the family, friends and advocates who are supporting this person.

What happens once the placement is accepted?

We produce a therapeutic care pathway that accompanies any placement offer. This document details information about Progress Housing, our assessment findings and our offer and recommendations related to placement within our services. The level of staff support and all costs are detailed.

Post Admission Review

As part of our caring and person-centred support, after six weeks a post-admission review takes place. This enables reflection on the settling in period for each person, and is also an opportunity to plan for longer term goals. We pride ourselves on being responsive and flexible to people’s needs so this review allows everyone to discuss the level of support and to ensure funding was set correctly. If any changes need to be made they are formalised in this meeting and formal requests made to relevant professionals. If things change, we change.

Each individual at our residential homes in West Sussex has their own unique and personalised plan reflecting their needs and aspirations. Through implementing this plan, each person is enabled to achieve their full potential and live a happy, richer life.

We are focused on providing the best possible opportunities for people to maximise their potential and the opportunities available to them. Our aim is for each person we support to be happy in our residential homes and live fulfilling richer lives.

We listen to each individual’s experience of our care homes, alongside their families and professionals supporting them, to ensure we are flexible and responsive to their needs. As a result of this approach, we believe our service is continually improving and adapting.

Our Framework for Quality Assurance means that we continuously evaluate the practices and services in our care homes in order to develop and improve.

Our Safeguarding Framework ensures open, honest and transparent reporting. Protection plans are produced to highlight specific areas in each person’s life that require particular attention. We are always quick to react to any incidents and learning is shared across all services. We strive to offer the best service possible to each person we support and believe our robust risk management process is second to none.

We support each individual’s development by using therapeutic interventions that are evidenced to aid with skill-building. All therapy undertaken is documented as part of each person’s care plan.

Our innovative Therapeutic Care Pathway model has been developed to provide each person we support with structured goals and outcomes.

Our support team have extensive experience at helping people with complex and varying needs. Their continual involvement ensures that assessment and interventions are the right thing for the individual and help each person to reach their goals.

Our staff undergo an excellent training programme and we invest in their development which results in an experienced and committed team.

Who we Support

We are experienced at providing the right specialist for each individual. The people we support have a wide range of needs including learning and physical disabilities, mental health needs, autism and Asperger’s and these complex needs require a specialist and tailored approach.

People with physical disabilities

We support people with the full range of daily living tasks, whether personal care, meal preparation, laundry, shopping, housework or managing finances and correspondence. In addition, we support people with physical disabilities to lead an active social life, both outside and inside the home.

People with Learning Disabilities

We understand that each person’s learning disability is unique to them and therefore requires an individual approach. From the point of referral, through to placement, and beyond, our focus is on ensuring each person is respected as an individual and is placed at the centre of our therapeutic approach.

People with Mental Health Needs

We support people with complex mental health requirements who also often have some form of learning or development need. One of the challenges in supporting people with a mental health need is maintaining stability in their health and avoiding fluctuating circumstances.

People with Autism and Asperger’s

We recognise that people with Autism or Asperger’s require a focused service with a specialist knowledge due to their specific needs. We place huge consideration on important elements like the nature and layout of any environment, noise levels and general sensory stimulus, as well as peer group, communication method, and the structure and pattern of each day.

People with Complex Behaviours

We are a provider of care to people with complex needs, which often manifest themselves through behaviours that are challenging. Although these behaviours can be challenging to the individual, their peers, their family or their supporting staff, we recognise that every behaviour is a form of communication and an expression of an individual’s need or personality.

People with Complex Health Needs

Within our services we support people who have a range of complex health needs. These are identified through referral and assessment where we consider carefully how we can meet the person’s specific needs.
